Project Request Form Project Request forms should be filled out by the requestor and signed by the department's Dean or Director, then sent (fax, email, or interoffice mail) to Campus Planning & Architecture, Mail Stop 5313, to be processed.
	Space Utilization Form 3.2M Fill out this form online to request new space or a change in how space is allocated between colleges or other major administrative units. Please print out form and send to the Space Utilization Manager for analysis and approval. Space Utilization Manager, Patrick Gorenchan, Bernhard Center Annex, Mail Stop 5215.
	Vendor Information & Bidder Application 72K (From the Office of Business and Finance) This form is to be used if an organization or company has not done business with the Purchasing Department at WMU. After filling out the form online, please print, then fax (269) 387-8824 or mail to: Western Michigan University, Purchasing Department, 1903 W. Michigan Ave., Kalamazoo, MI 49008-5342.
